Please read carefully:
I understand that massage therapy provided by Golden Sanctuary Massage is for relaxation, stress reduction, and relief of muscular tension only.
I understand that massage therapy is not a substitute for medical care, diagnosis, or treatment. The massage therapist does not diagnose illness, prescribe medications, or perform spinal adjustments.
I affirm that I have disclosed all known medical conditions and injuries and will update the therapist of any changes.
I understand that it is my responsibility to communicate discomfort during the session so adjustments can be made.
I release and hold harmless Golden Sanctuary Massage and its therapist from any liability arising from failure to disclose medical conditions or from my participation in massage therapy services.
I understand that draping will be used at all times and that I may stop the session at any time.
I understand that inappropriate behavior will result in immediate termination of the session.
Appointment & Cancellation Policy
Golden Sanctuary Massage
I understand that my appointment time is reserved specifically for me. Golden Sanctuary Massage requires a minimum of 24 hours’ notice for cancellations or rescheduling.
• Cancellations with less than 24 hours’ notice may be charged 50% of the scheduled service price.
• Same-day cancellations and no-shows may be charged 100% of the service price.
• Clients arriving more than 15 minutes late may have their session shortened to avoid impacting the next client and will still be charged the full-service price.
